What does a care coordinator do?

A care coordinator helps your family develop, implement, and monitor care arrangements for elderly or disabled loved ones. A care coordinator is involved in all discussions about finding and paying for care, helping you weigh the financial implications of each care decision.  Services include:

  • Conducting assessments to identify needs and develop appropriate care plans
  • Helping families find resources and community services
  • Monitoring in-home health or companion services
  • Acting as a liaison between the elderly or disabled loved one and family members living at a distance. In addition to monitoring the loved one’s situation, our firm provides a local presence who has detailed knowledge about local service providers and community resources.
  • Transitioning the elder to a new living situation including a retirement community, assisted living or a skilled nursing facility
  • Reviewing care issues and referrals to appropriate community resources as necessary

Our parents want to live at home but it doesn’t seem safe. How can you help us?
It starts with an evaluation of your parents’ home as well as their health status. That assessment tells us whether it’s possible to modify their home to make it safe. It also will help us determine whether home care services might be appropriate and what options exist to pay for that care.

We were told that our mother may need nursing home care. How can we be sure it’s right for her?
We deal with many clients who are confronted with the same issue. Many elderly people have heard horror stories of nursing homes, and thus are biased against them. Our goal is to always keep people in the least restrictive environment possible while maintaining their safety.  But we also weigh the potential benefits the nursing home may provide, such as socialization and safety. We work with many nursing homes, so we can suggest which ones may be better options for your Mom, but in the end the choice is always yours.

Is a nursing home the only option if a parent can’t stay home?
There are many other options available. All provide different levels of care. Payment options also vary. Some facilities are private pay, meaning you pay out of pocket. Others accept patients on Medicaid or other public benefit programs. There are many options and many variables, which makes it very difficult for the average person to know what to do next.
